Drain the right space and protect function.
- Define whether the abscess is perianal, ischiorectal, intersphincteric or supralevator
- Drain dependent infected spaces adequately
- Look for horseshoe extension when clinically suspected
- Avoid unnecessary sphincter injury
- Recognize fistula disease when recurrence or complex anatomy is present
