Handle it like a lesion that may be neoplastic.
- Avoid grasping or rupturing the distended appendix
- Inspect the base and cecum
- Assess for mucin or peritoneal implants
- Resect with negative gross margins as anatomy permits
- Send intact specimen for pathology and tailor further surgery to pathology
